Support Coordinator
Location: Port Lincoln & Eyre Peninsula
Employment Type: Full-time
Salary: Negotiable based on experience
Empowrd is seeking a dedicated and compassionate Support Coordinator to join our dynamic team and help expand our services across the Eyre Peninsula. If you enjoy interacting closely with others, have experience working within the disability sector and/or lived experience, and can work flexibly alongside a small team, perhaps you should consider applying for this role.
Key Responsibilities
- Working closely with clients to identify their needs, personalise support and ensure they are receiving appropriate services that align with their NDIS goals and aspirations.
- Promoting and fostering independent decision-making, including choice and control.
- Coordinating and liaising with service providers, and reviewing goals and jointly determining new ones with participants.
- Engage in NDIS plan re-assessments and plan reviews.
- Empower clients to become active participants in their own recovery process and decision-making.
- Prepare regular reports and summaries of participant progress for internal and external stakeholders.
What We’re Looking For
- Knowledge of NDIS is desirable but not essential.
- Excellent organisational and communication skills, written and verbal are essential.
- Strong interpersonal skills with the ability to establish rapport, build trust and maintain boundaries is essential.
- Empathy, compassion, and a non-judgmental attitude.
- Ethical and culturally sensitive approach.
- Initiative to work and solve problems unsupervised and independently, with a strong focus on reaching set goals, targets and KPI’s.
- Ability to liaise effectively with participants, community groups, employees, external agencies, government departments and other stakeholders.
- Ability to use customised software, case note management and report writing.
- Ability to maintain accurate and up-to-date client records, progress notes, and documentation of actions.
- Valid drivers licence.
- Valid Working with Children Check for paid employment.
- Valid NDIS Worker Screening Check (or willingness to obtain)
- 3 valid referees to confirm the essential key points
